Yes, SMP can be worthwhile even if you already enjoy shaving your head. It does not create hair or stop hair loss. Its purpose is to reduce the contrast between scalp and hair, define a natural-looking hairline and make a shaved style appear more deliberate, even and consistent.
What can SMP add to a shaved head?
If you are already comfortable shaving, you have solved one important part of hair loss: you have found a style that suits you. Scalp micropigmentation may still be worth considering if you want to refine how that style looks rather than change it.
SMP places carefully matched pigment into the scalp to imitate the appearance of closely cropped follicles. On a shaved head, this can soften the difference between your skin and the areas where hair has receded or disappeared. It can also create the visual impression of a more consistent buzzed shadow across the scalp.
For some men, the main benefit is a more defined hairline. For others, it is reducing the visibility of a bald crown, uneven density or scars. The result is not a new head of hair. It is a cosmetic treatment designed to make a shaved appearance look intentional and balanced.

Why get SMP if you are already bald, and is it good for bald men?
A completely shaved scalp can still show differences in tone and texture. Areas with hair follicles may have a darker shadow, while advanced hair loss, scars or a receding hairline can leave more visible skin. SMP works with those contrasts rather than trying to disguise the fact that you shave.
It can be particularly useful when you want:
- a more even-looking follicle shadow across the scalp
- a natural-looking hairline that suits your face
- less visual emphasis on a bald crown or receded temples
- camouflage for some types of scalp scarring
- a consistent appearance between shaves
It can also make the daily result of shaving feel more predictable. Without SMP, the appearance changes as stubble grows, depending on lighting and the contrast between your skin and remaining hair. With SMP, the pigment remains as a visual foundation, although your natural hair and skin still change over time.
That does not mean every shaved man needs treatment. If you like the natural look of your scalp and are not bothered by uneven density, there may be no practical reason to have SMP. I would rather tell you that honestly than suggest a procedure that does not solve a problem for you.
Will SMP make a shaved head look like it has hair?
SMP can make a shaved head look as though it has very short follicle shadow, but it does not produce physical hair. It cannot create texture, length or genuine growth. Someone running their hand over the scalp will still feel skin, not stubble created by the treatment.
The most convincing plan is usually one that works with your normal shaving routine and your natural features. I assess your existing hairline, skin tone, scalp condition, facial proportions and the pattern of your hair loss before recommending a design. A hairline that is too low, too straight or too dark can look less natural, so restraint matters.
Colour matching is also more careful than choosing a generic black shade. Pigment is colour-matched to your skin and existing hair so the follicles read as subtle points rather than solid marks.
Do you need to keep shaving after SMP?
Most men choosing the shaved style continue to keep their remaining hair short. This helps the treated areas and natural follicles blend together. You may use an electric clipper or a razor, depending on the finish you prefer. SMP does not remove the need to shave if you want a clean, closely cropped appearance.
Your routine may feel easier because the scalp already has a consistent visual base. However, you still need to protect the skin and follow aftercare after each session. At Foli Sim, we do not use anaesthetic and advise against it. I will explain what to expect and how to care for the treated area.
Aftercare includes keeping the scalp dry for the initial healing period, washing gently once advised, avoiding sun exposure while the skin settles and using SPF 50 once appropriate. Swimming pools should be avoided for a period after the final session, and your practitioner will tell you how long. Your practitioner will give you instructions suited to your treatment.
Is SMP a permanent decision?
SMP is a long-term cosmetic treatment, so it deserves proper consideration. Pigment gradually softens over the years and may need a touch-up. Your scalp, skin and hair loss can also change. A good design should allow for those changes rather than assume your appearance will remain exactly the same.
If you are worried about regretting the decision, think about whether you already feel comfortable with a shaved look. That is usually a helpful starting point. SMP is less likely to feel like a dramatic change when it supports a style you already wear. The bigger decision is whether you want to add a designed follicle shadow and hairline to that style.

How do you know if SMP is worthwhile for you?
I suggest asking yourself three questions. First, do you enjoy the shaved look, or are you only shaving because you feel you have no alternative? Second, what specifically bothers you, the hairline, the crown, uneven tone, scars or the contrast between skin and remaining hair? Third, would a subtle improvement be enough, or are you expecting the treatment to recreate hair?

Can SMP cover a completely smooth bald scalp?
SMP can be used on areas with advanced hair loss, but the plan depends on your scalp, skin, desired hairline and the amount of natural follicle shadow remaining. The aim is to imitate closely shaved follicles, not to create real hair. An assessment is needed to decide how natural and balanced the result can be.
Will people be able to tell that I have had SMP?
A well-planned result is intended to look like closely cropped follicles rather than obvious markings. Visibility depends on pigment selection, density, hairline design, skin tone, lighting and how closely you shave. I discuss realistic expectations during consultation, including the situations in which the treatment may be more noticeable.
Can SMP hide a scar on my shaved head?
SMP may be suitable for some scalp scars, including scars associated with hair loss or previous procedures. Scar tissue can accept pigment differently from normal skin, so the area needs to be assessed separately. Foli Sim offers scar treatment, but I would only recommend it after examining the scar and surrounding scalp.
Does SMP hurt without numbing cream?
Foli Sim does not use anaesthetic and advises against it. Sensitivity varies between people and across the scalp, so your practitioner will explain what the treatment feels like and how sessions are managed. If you have particular concerns about discomfort or skin sensitivity, raise them before proceeding.
Can I change the hairline later if my hair loss continues?
Hair loss can continue after SMP, which is why the design should allow for future change. Pigment can soften over time and touch-ups may be considered, but changes are not something to assume can always be corrected simply. Your consultation should include an honest discussion of your current and possible future pattern.
Is SMP suitable if I sometimes grow my hair longer?
SMP for a shaved style is designed to blend with very short hair. If you regularly grow your hair longer, the visual effect may not suit every length or hairstyle. Foli Sim also offers density work for unshaved hair, which creates a softer result between existing hairs rather than the defined look of shaved SMP.
How soon can I return to normal activities after SMP?
You will need to follow aftercare while the scalp heals. This includes keeping it dry initially, washing gently when advised, avoiding sun exposure during early healing and delaying swimming as instructed.
